Scene in the Original Work:
In the original manga, Tatsuya has carried his late twin brother Kazuya's dream and led the team to Koshien. On the day of the opening ceremony, instead of attending, he meets Minami at the riverbank and confesses with this line, using his own full name and hers. In the TV anime, he says it to her over the phone after arriving in Hyogo, and the series ends there.
