Original Scene & Cultural Context
Scene in the Original Work:
Near the end of the 1988 film, Kaneda goes alone to stop his childhood friend Tetsuo, who has awakened psychic powers and gone on a rampage, and aims a laser rifle at him. Tetsuo lets out that he always hated Kaneda bossing him around, and shouts "Kanedaaa!" Kaneda answers with this line, and Tetsuo screams "Die!" and attacks.

Cultural Context:
AKIRA is a manga by Katsuhiro Otomo, who also directed the 1988 anime film. This line comes from the film and is said not to appear in the original manga. デコ助 is old Tokyo slang teasing someone with a big forehead. Kaneda and his friends usually call each other by name without さん, so the line is seen less as a point of manners than as Kaneda pushing back against Tetsuo and breaking with him. It is known as a joke reply when someone leaves さん off a name.

Trap for English Speakers
Calling someone by name without さん is 呼び捨て. Among close friends it is normal and friendly, but toward a senior or a stranger it is rude. 野郎 and デコ助 are strong insults; never say them to a real person except as an obvious joke.
